Hope & Care Community Services is a grass-roots community services organisation with nearly a decade of experience in Brisbane. We are a registered NDIS provider and community services organisation that works with people of all ages — including people with disability, older Australians, young people transitioning from residential care and youth justice settings, individuals being discharged from hospital, and members of the broader community.

Our Community Workers connect people to services, coordinate with DCSSDS, NDIA, and Queensland Health, develop community programmes, and advocate for the people in our care.
