Hope & Care Community Services provides support coordination to help NDIS participants understand their plans, connect with suitable providers and use their funded supports effectively. Our approach promotes informed choice, confidence and greater independence.

Support Coordination We Provide

Depending on participant needs and service availability, support may include:

  • Understanding NDIS plans and support budgets
  • Connecting with suitable providers and community services
  • Coordinating different supports and service agreements
  • Building skills to manage supports independently
  • Preparing for changes or challenges in support arrangements
  • Reviewing whether current supports are meeting participant goals
  • Providing progress information and reports when required

Who Can Access Support Coordination?

Support coordination may be available when it is funded in a participant’s NDIS plan. The level of assistance provided depends on the participant’s circumstances, support needs and approved funding.

A support coordinator works with the participant to organise supports around their individual needs, circumstances, goals and available funding.

Levels of
Support Coordination

The NDIS may fund three levels of support coordination:

Level 1: Support Connection – Assistance to understand the plan and connect with providers and community supports.

Level 2: Coordination of Supports – Support to coordinate services, build capacity and increase independence.

Level 3: Specialist Support Coordination – Higher-level assistance for participants with complex needs or barriers to accessing supports.
